Client Consent Agreement

By booking a session with RishtaCare, I understand and agree that:

• Counselling is a supportive and collaborative process.
• Results cannot be guaranteed.
• I am voluntarily participating.
• I am responsible for my decisions and actions.
• RishtaCare does not provide psychiatric diagnosis or medical treatment.
• In case of crisis (self-harm, violence risk), I will seek emergency services.
• Sessions are confidential except where disclosure is required by law.
• I will not record sessions without written permission.
• I confirm that I have read and understood this agreement.
